• Missed or canceled appointments without 24-hour notice will result in a $75 charge.
• First-time clients MUST complete a prescreening consent form or it will be canceled.  A link to the form will be in your confirmation email. Please check your spam or junk folder if you don't receive it upon booking.  

Note: Client check-in appointments without a purchase, monthly series, or package will be charged a $40 fee.

Pricing is subject to increase pending dosage. ONLY refundable with abnormal thyroid testing. Administered pending consultation.

Dietary Coaching Sessions

IMPORTANT:

Bloodwork is required before starting on all services except Lipotropic Injections and Dietary Coaching.  Once you complete the consent form, FMW will contact you before your appointment to discuss the acceptable forms of bloodwork listed below:                         

1. You can provide a recent copy of your bloodwork if collected within the last 6 months. 

2. Our nurse practitioner can provide a lab order for you to go to LabCorp. Lab processing fees are not included.
